Please submit this form for chemical waste only. List each waste container separately.

If there are multiple containers with the same waste contents, you may enter one container and then duplicate it. Enter the details of the waste in the container field, select "Actions" in the upper-right-hand corner of the field, then select "Duplicate Container" as many times as needed.
Waste containers that are not secured with a proper cap, leaking, or properly labeled are unacceptable for pickup by EHS.
For other wastes (biological, radiological, etc.) see the TWU EH&S Waste webpage for their respective pickup requests. If you have mixed waste, contact EH&S for guidance on which form to complete.
