Please submit this form for radioactive wastes only (liquid, solid, sealed sources, mixed waste, etc.). List each waste container separately with the exception of scintillation vials. Scintillation vials can be disposed of together in a box but should be separated by isotope.
For the "Container Contents" Section, list the isotope, ~ activity in mCi (waste container log will have the specific activity for the container), and any other constituents (e.g. eco-safe scintillation fluid).
For other wastes (chemical, biological, etc.), see the TWU EH&S Waste webpage for their respective pickup requests. If you have mixed waste, contact EH&S for guidance on which form to complete.
